颈椎多体位磁共振成像的初步研究

摘    要:
目的:采用颈椎前屈、后伸及中性自然位等多体位进行颈椎磁共振成像,探讨高场磁共振颈椎多体位成像在诊断颈椎病中的价值。方法:采用1.5T高场磁共振进行颈椎前屈位、中性自然位及后伸位成像,分析比较不同体位下颈椎序列、椎间盘及椎管等的变化情况。结果:30例受检者随屈伸体位的变化,颈椎序列不稳加重。前屈位新出现突出椎间盘17.33%(26个/150个),较中性自然位突出加重62.50%(35个/56个);后伸位检查较中性自然位新出现椎间盘突出13.33%(20个/150个),加重58.93%(33个/56个)。前屈位较中位以硬膜囊前间隙狭窄变化为著,后伸位检查时硬膜囊后间隙狭窄度加重。结论:颈椎多体位磁共振检查提供了一种更为全面而准确的非侵袭性颈椎检查方式,可以早期显示潜在的颈椎病致病因素。

Abstract:
Objective:To study the clinical value of multi-position MRI and to obtain early precise diagnosis of cervical spondylosis.Methods:The cervical spines of 30 cervical patients were examined in 1.5T MR at multi-position: flexion position,neutral position,extension position.The difference of cervical spines of MR imaging were to statistics analyses.Results:The instability of cervical sequence increased by flexion and extension position. 26 intervertebral discs(17.33%) were emerging and 35 intervertebral discs(62.50%) were aggravating.20 preliminary study of multi-position MR imaging of cervical vertebra intervertebral discs(13.33%) were emerging and 33 intervertebral discs(58.93%) were aggravating.13 patients narrow of dural sac post diastema were aggravating in extension position.Conclusion:The multi-position MRI of cervical spine could provid a non-invasive all round and precise examination,to obtain early precise diagnosis of cervical spondylosis from appearance and function.
