关节镜清理术治疗膝骨性关节炎 56 例的临床研究

目的：探讨膝关节镜关节清理术对膝关节骨性关节炎的疗效。方法：2006年4月至2012年5月对56例（59膝）膝关节骨性关节炎患者应用关节镜行关节清理术。术前和术后第6、12、24月观察膝关节疼痛、肿胀、关节功能情况并进行膝关节评分。结果：本组有53例（55膝）患者获得随访，随访时间6-24月。所有随访患者术后1~2月疼痛、肿胀明显缓解。无感染、无血肿形成、无深静脉栓塞。膝关节综合评分：术前与术后6、12、24月优良率的比较，差异均有统计学意义（P〈0．05），术后2年内优良率大于70％，但有逐年下降趋势（P〈0．05）。结论：膝关节镜下关节清理术对膝关节炎早期病变有较好的近期疗效。

Clinical study of 56 cases of arthroscopic debridement for osteoarthritis of knee

Objective： To From Apr 2006 to May 2012 investigate the curative effect of arthroscopy on osteoarthritis of knee. Methods： , 56 patients （59 knees） underwent arthroscopic surgery for osteoarthritis of knee： 24 males and 32 females, 49 to 72 years old,averaged 63 single knee 53 cases , double knees 3 cases. Classification by X-rat： grade Ⅰ 30 knees, grade Ⅱ 22 knees, grade Ⅲ 7 knees ,grade Ⅳ4 knees. Clinical signs such as pain, swelling, and knee function were observed and assessed by knee joint synthetical score system before and 6,12, and 24 months after surgery. Results： 53 patients（55 knees） were followed-up for 6 to 24 months, 1-2 months after the surgery the pain was significantly relieved in all the follow-up patients, no infection hematoma, or vein thrombosis was found. The excellent-fine rate of knee joint synthetic score 6,12 and 24 months postoperation all were significantly higher than that of preoperation （P 〈 0.05）. Conclusion： Arthroseopic debridement has good near-term efficacy for early leasion of osteoarthritis of knee.
