37例甲状腺乳头状微小癌临床病理特点分析

目的：探讨甲状腺乳头状微小癌（papillary thyroid microcarcinoma，PTMc）的临床病理特点。方法：对经手术切除的37例PTMC的临床病理资料进行回顾性分析。结果：37例PTMC中男性6例，女性31例，年龄17～69岁，癌肿大小0．1～1．0cm。有30例是在甲状腺良性疾病中发现的。CK19、HBME～1、Galectin-3、34βE12在PTMC中阳性表达率分别为100％、91．3％、95．7％、87．0％，Ki-67在PTMC中均有表达，阳性细胞比例为1％～5％。结论：PTMC好发于女性，常与其他甲状腺良性疾病并存，病理医生应准确把握PTMC的镜下特征，必要时可联合免疫组化以辅助诊断。

Clinical and histopathological features of 37 cases of papillary thyroid microcarcinoma

Objective： To investigate the clinical and histopathological features of papillary thyroid microcarcinoma （PTMC）. Methods： 37 cases of papillary thyroid microcarcinoma were treated by surgery and their clinical and histopathological features were analyzed retrospectively. Results： Of the 37 cases, male 6 cases, female 31 cases, aged 17 to 69 years. The size of tumors were from 0.1cm to 1 cm. 30 cases of PTMC were detected in patients with benign thyroid tumor. The positive expression rate of C K 19, HBME-l, Galectin-3 and 34βE 12 in PTMC was 100 %, 91.3 %, 95.7 % and 87 % respectively. Ki-67 was expressed in all PTMC and the rate of positive cells was 1%~5 %.Conclusion： The incidence of PTMC is high in females, the PTMC often coexists with benign tumor of thyroid.Pathologist should master the histopathologieal features accurately. Immunohistochemical staining can be combined for differential diagnosis.
