戊酸雌二醇在重度宫腔粘连分离术后的应用

目的:探讨大剂量雌激素对重度宫腔(IUA)粘连预后的影响及其对凝血功能、肝功能、血脂的影响。方法:重度IUA患者68例,随机分为实验组及对照组各34例,宫腔镜IUA分离术术后辅助大剂量雌激素(戊酸雌二醇9mg·d-1)治疗。实验组给予戊酸雌二醇9mg·d-1,连续6m。对照组给予补佳乐9mg·d-1,共21d,后10d加服安宫黄体酮8mg·d-1,共6m。术后l、3、6m随访疗效及凝血功能、血脂、肝功能的变化。结果:实验组的有效率(50%)明显高于对照组(35.29%),差异有统计学意义(P0.05)。服药前及服药后1、3、6m患者的凝血功能、血脂、肝功能的监测结果无统计学差异。结论:在重度IUA术后连续应用大剂量戊酸雌二醇的预后优于其周期性应用。对凝血功能、血脂、肝功能无不良影响。

Application of estradiol valerate in severe intrauterine adhesions after hysteroscopic adhesiolysis

Objective：To investigate the effect of high dose estrogen in prognosis of severe intrauterine adhesion（IUA）and its impact on coagulation function and liver function,blood serum lipids.Methods：68patients with severe IUA underwent hysteroscopic adhesiolysis were randomly divided into study group and control group evenly.The patients in study group were applied estradiol valerate 9mg d^-1 for 6m;the patients in control group were applied estradiol valerate 9mg d^-1 for 21d,medroxyprogesterone 8mg d^-1 with the last 10das one period,and taking three periods.The efficacy rate was assessed and the changes of coagulation,blood serum lipids,liver function l,3and 6month after treatment were recorded.Results：The efficacy rate in the study group（50%）was significantly higher than the control group（35.29%）.There was no significant alteration in the coagulation,liver f′unction or blood serum lipids 1,3and 6months after treatment.Conclusion：Patients with severe IUA treated continuously with high dose estradiol valerate had advantage than periodically.There is no adverse effect on hlood coagulation fumition,liver function or blood serum lipids of high dose estradiol valerate.
