踝关节骨折脱位的手术治疗

目的 总结治疗踝关节骨折脱位的手术方法和治疗效果。方法 对1995年～2002年手术治疗踝关节骨折脱位98例进行分析。男66例，女32例；根据Weber-AO分类B型45例，C型53例。应用改良的Baird和Jacksorn评分系统对其术后疗效进行评定，并将骨折类型、骨折固定方法、术后功能锻炼时间与最终功能的关系进行分析。结果 随访时间为3-55个月，平均16个月，98例疗效为：优61例，良27例。可9例，差1例，优良率为89．8％。结论 手术治疗踝关节骨折脱位可取得满意的骨折复位和临床疗效，复位的质量与疗效密切相关，判断骨折类型和选择固定方法对于提高复位质量，改善远期疗效十分重要。

Surgical Treatment of Ankle Fracture and Dislocation

Objective To summarize the surgical techniques and results of operational reduction and internal fixation performed for ankle fracture and dislocation.Methods To analyze the Ninety-eight patients cases with ankle fracture and dislocation,who were treated with open reduction and internal fixation from 1995 to 2002.The patient group consisted of 66 males and 32 females.According to Weber-AO classification,the group was made up of 45 Weber B cases ,53 Weber C Cases.All patients were evaluated by modified Baird and Jackson scoring system.The relationship between final result and fracture pattern,fixation methods,the time of exercise initiation after operation were analyzed.Results The followed-up period varied from 3 months to 55 months,with an average of 16 months.The number of patients whose results was excellent,good,fair and poor was respectively 61,27,9 and 1.The total percentage of good to excellent clinical results was 89.8%.Conclusions Operative treatment may provide satisfactory fracture reduction and clinical results for ankle fracture and dislocation.Correct fracture pattern estimation and proper internal fixation is of importance to achieve and maintain reduction,and to gain better long-term result.
