针型与普通胸腔镜手术治疗自发性气胸的对比分析

目的探讨并对比针型胸腔镜与普通胸腔镜治疗自发性气胸的手术方法和临床效果。方法2003年12月～2005年5月65例行胸腔镜手术治疗的自发气胸患者,其中行针型胸腔镜手术治疗27例,普通胸腔镜手术38例,比较两组术后恢复情况及术后并发症。结果两组手术时间、胸腔引流管拔除时间、术后残余漏气差异均无显著性（P〉0.05）,但是术中失血量、患者住院时间、术后患者肋间皮肤不适感方面,针型胸腔镜组明显少于普通胸腔镜组（P〈0.05）;术后1个月针型胸腔镜手术瘢痕不明显。结论针型胸腔镜手术是自发性气胸更为安全有效的术式,优于普通胸腔镜。

A comparative analysis of needlescopic versus conventional video-assisted thoracic surgery for primary spontaneous pneumothorax

Objectives] To compare and analyze surgical methods and clinical effects of needlescopic versus conventional video-assisted thoracic surgery(VATS)in the treatment for primary spontaneous pneumothorax.Methords] From Deceber 2003 to May 2005,65 cases with primary spontaneous pneumothorax were operated by either needlescopic or conventional VATS in our hospitals and their surgical managements and postoperative complications were compared and analyzed herein.Among them,27 cases were performed by needlescopic VATS while 38 cases by conventional VATS.Results] All patients were cured.There were no statistical differences between needlescopic versus conventional VATS with the operating time,persisting time of thoracic drainage and postoperative remaining air in the thorax.With the operative bleeding,hospitalized time and the paients' skin incision and scar,however,needlescopic VATS was better than versus conventional VATS and there were statistical differences(P <0.05).Conclusion] In the treatment of primary spontaneous pneumothorax,needlescopic VATS is more effective and safe than the conventional VATS.
