三联手术治疗老年白内障并急性闭角型青光眼的临床分析

目的探讨三联手术对老年白内障并发急性闭角型青光眼的患者的临床意义。方法对40例（40只眼，其中男21眼，女19眼）有白内障并发急性闭角型青光眼的患者，都实施白内障超声乳化吸出＋人工晶体植入＋青光眼小梁切除术（简称为三联手术），于术前和术后1周和1个月，检测测视力、眼压及前房深度。结果40眼中术前与术后1周视力比较，术后视力较术前有明显提高。术前前房深度（ACD），与术后1周比较（P〈0．01），房角明显增宽。术前与术后1周眼压比较（P〈0．01），术后眼压明显低于术前。结论老年白内障并发急性闭角型青光眼的患者实施白内障超声乳化吸出＋人工晶体植入＋青光眼小梁切除术的三联手术，能有效地提高视力，是一种安全有效的手术方法。

Surgical Treatment of Senile Cataract and Acute Angle - Closure Glaucoma

Objective To investigate the clinical significance of triple cataract surgery in elderly acute angle -closure glaucoma. Methods Totally 40 patients （40 eyes, 21 eyes of male and female 19） with cataract and acute angle- closure glaucoma patients, all of cataract phacoemulsification ＋ intraocular lens implantation ＋ trabeculectomy （referred to as the three joint surgery）, preoperative and postoperative 1 week and 1 month, testing visual acuity test, intraocular pressure and anterior chamber depth. The results of 40 eyes of the preoperative and postoperative visual acuity between 1 week and postoperative visual acuity improved markedly. Anterior chamber depth （ACD）, and 1 week after surgery compared （P 〈 0.01 ） , anterior chamber angle was widened. Preoperative IOP after 1 week compared with （P 〈 0.01 ） , significantly lower than the preoperative IOP. Conclusion Senile cataract and acute angle - closure glaucoma in patients of cataract phacoemulsification ＋ intraocular lens implantation ＋ trabeculectomy glaucoma triple procedure, can improve vision, is a safe and effective method.
