介入疗法在支气管结核治疗中的作用

目的探讨介入疗法在气管、支气管结核治疗中的应用价值。方法 60例支气管结核患者随机分为治疗组(n=30例)和对照组(n=30例),2组抗结核治疗方案均为3HRZE(S)/9HRE,治疗组施以介入治疗。结果介入治疗2个月后痰培养结核分枝杆菌阴性率为94.4%(17/18),治疗12个月达100%(18/18)。X线影像学结果显示,介入治疗后3个月病灶吸收显效率为40.0%(12/30),有效率为60.0%(18/30);治疗12个月显效率为96.7%(29/30),有效率为3.3%(1/30)。空洞闭合率介入治疗后3个月为91.7%(11/12),治疗12个月达100%(12/12)。治疗结束时临床症状消失率为100%。结论应用纤支镜对气管支气管结核患者进行介入治疗,能提高治愈率,是一种安全有效的方法。

Interventional therapy in bronchial tuberculosis

Objective To evaluate the value of interventional therapy in the treatment of tracheal and bronchial tuberculosis.Methods 60 cases with endobronchial tuberculosis were randomly divided into a therapeutic group（group A,30 cases） or a control group（group B,30 cases） which both groups were treated with 3HRZE（S）/9HRE.Group A was treated with interventional therapy.Results The sputum culture negative conversion rate was 94.4%（17/18）after 2 months＇ interventional therapy.The rate increased to 100%（18/18）after 12 months＇ therapy.Chest X-ray showed that the foci were markedly absorbed in 40% cases（12/30）,and the effective rate was 60%（18/30）after 3 months＇ interventional therapy and it increased to 96.7%（29/30）,and the effective rate was 3.3%（1/30）.The cavity closure rate was 91.7%（11/12） after 3 months＇ interventional therapy and was 100%（12/12） after 12 months.The rate of symptom disappearance was 100%.Conclusion Interventional therapy with bronchoscopy for endobronchial tuberculosis is a safe and effective method.
