大肠癌并急腹症的诊治(附37例报告)

目的:探讨大肠癌并急腹症的临床特点及诊治方法。方法:回顾性分析37例大肠癌并急腹症的临床资料。结果:入院诊断肠梗阻24例,肠穿孔4例,急性阑尾炎3例,下消化道出血2例,腹痛待查4例。急诊手术23例,择期手术12例,根治性手术16例,姑息性手术7例,短路手术3例,结肠造口6例。手术死亡2例,肠瘘2例。7例左半结肠癌急诊行根治或姑息切除,Ⅰ期肠吻合术,恢复顺利。结论:大肠癌并急腹症以肠梗阻最多,肠穿孔及出血次之;老年急腹症患者入院时多怀疑肿瘤存在,但确诊困难。无论左、右半结肠癌,急诊手术时均应积极行根治切除并Ⅰ期吻合术,预后满意。

Diagnosis and treatment for colorectal carcinomas combined with acute abdomen complications: a repot of 37 cases

Objective:To analyze the clinical feature and the methods of diagnosis and treatment for colorectal carcinomas combined with acute abdomen complication. Methods: To review and analyze the clinical data of 37 cases who were colorectal carcinomas combined with acute abdomen complication. Results: Among 37 cases 24 cases were diaguozed as intestinal obstruction, 4 cases as intestinal perforation, 3 cases acute appendicitis, 2 cases lower digestive organ hemorrhage, 4 cases bellyache yet to be investigated. Among these cases, 23 cases received emergency operation, 12 cases selective operation, 16 cases radical operation, 8 cases palliative operation, 3 cases short-circuit operation, 6 cases artificial colon opening. 2 cases dead dae to operation 2 cases deceloped intestine fistula. Conclusion: The acute abdomen complications in colorectal carcinomas patients are intestine obstruction, intestinal perforation, lower digestive organ hemorrhage, etc.
