颈前路椎体次全切除钛附内固定治疗脊髓型颈椎病

目的探讨前路颈椎体次全切除钛网植骨内固定治疗脊髓型颈椎病的临床疗效。方法回顾我院2007年1月～2010年12月期间诊治脊髓型颈椎病患者18例，进行颈前路椎体次全切除减压术和前路钛网植骨内固定术。结果随访12个月，18例患者在6个月时植骨均全部融合，无钛网移位脱落或螺钉松动，神经功能得到了不同程度恢复。根据JOA分级方法对脊髓功能评分，术后患者脊髓功能均有不同程度好转或恢复。其中优14例（77．78％），良3例（16．67％），中1例（5．55％）。术前平均（9．89±1．62）分，术后平均（16．0±1．28）分，差异有统计学意义（P〈0．05）。结论应用颈前路椎体次全切除钛网植骨内固定术治疗脊髓型颈椎病是安全有效的。

The treatment of cervical spondylosis myelopathy by anterior cervical mostly vertebrectomy technique and titanium mesh cage bone fusion and internal fixation

Objective To analyze the clinical effects of the treatment of cervical spondylosis myelopathy by anterior cervical mostly vertebrectomy technique and titanium mesh cage bone fusion and internal fixation.Methods Reviewed the treatment of 18 cases who with cervical spondylosis myelopathy in January 2007 to December 2010 by anterior cervical mostly vertebrectomy technique and titanium mesh cage bone fusion and internal fixation.Results With an endpoint of 12 months,at the 6-month follow-up examination,all the 18 patients of bone planted were in fusion.Titanium mesh shedding,screw loose and plate breaking were not found.The function of nerve had been improved.According to the JOA classification method for spinal cord functional score,postoperative function in patients with spinal cord were different degree better or recovery.Among them 14 cases(77.78%) were perfect,3 cases(16.67%) were good,1 case(5.55%) was medium.The preoperative average was(9.89±1.62),and the postoperation average was(16.0±1.28),the difference of preoperative and postoperative was statistically significant(P < 0.05).Conclusion It is effective and safe of anterior cervical mostly vertebrectomy technique and titanium mesh cage bone fusion and internal fixation in cervical spondylosis myelopathy.
