清肝泻心、滋阴润燥法治疗2型糖尿病心肝郁热证临床研究

目的探讨清肝泻心、滋阴润燥法及两法合用对2型糖尿病心肝郁热证的疗效。方法将本病患者100例随机分为清肝泻心、滋阴润燥法组(第1组)、清肝泻心法组(第2组)、滋阴润燥法组(第3组)和达美康组(第4组),每组各25例。第4组单用达美康,第1、2、3组均以达美康作为基础治疗,加服相应的中药,疗程均为4周。各组均测试治疗前后的餐前、餐后血糖(FBG、PBG)及空腹胰岛素(FINS)、C肽(C-P)、糖化血清蛋白(GSP),计算胰岛素敏感性指数(IAI)。结果(1)第1组降糖疗效总有效率为92.00%,与其它各组的疗效相当(P>0.1);(2)第1组改善中医症状疗效总有效率为92.00%,在总显效率方面优于其它各组(P<0.05);(3)第1、2、3组提高IAI作用显著(P<0.01),且第1组的作用优于第2、3组(P<0.01)。结论清肝泻心、滋阴润燥法治疗2型糖尿病可能通过控制高胰岛素血症、增强机体对胰岛素的敏感性等胰外作用途径,达到减轻外周胰岛素抵抗而降低血糖的目的。

Clinical Study on Method of Eliminating Liver - Fire, Removing Excess Fire of Heart and Nourishing Yin to Relieve Symptoms of Dryness Treating Type 2 Diabetes with Heart and Liver Fire Caused by Blood Stasis

Objective:To explore the clinical effects of Eliminating Liver -Fire,Removing Excess Fire of Heart(method E )and Nouishing Yin to Relieve Symptom s of Dry-ness(method N)and that of both of them when being use d together in treating type2diabests mellitus,at the asme time to evaluate the clinical differences of them one by one.Methods :100patients with type 2diabetes were divided randomly into 4groups with 25patients each.T hey are with method E and N (group 1)?method E (group 2)?method N(group 3)and Diamicron(group 4).Before and affer treatment for 4weeks,the leve ls of FBG?PBG?FINS?C -P ?GSP?IAI were examined in all the patients.Results:(1)The total effective rate of reducing blood sugar in group 1was 92.00%,which was equivalent to levels in other groups.(2)In group 1,the total effective and significant rates of releaing syndrom e in TCM were better than other group.(3)In group 1?2?3,the IAI was signficaltyly in-creased,but the group 1was more sign ificant.Conclusion:It suggested that the treatment of Eliminating Liver -Fire,Removing E xcess Fire of Heart(method N)and Nouishing Yin to Relieve Symptom s of Dryness(method E )might decrease insulin peripheral resistance and r educe blood sugar by means of reducin g hy-perinsulinemia and improving insul in sensitivity.
