真菌性鼻窦炎25例CT诊断分析

目的探讨真菌性鼻窦炎的CT影像学特征。方法回顾性分析经病理证实的25例真菌性鼻窦炎患者的临床与CT影像资料。结果本组25例真菌性鼻窦炎中由曲霉菌感染引起21例,表现为真菌球形成,CT表现为:(1)病变鼻窦多为单窦,以上颌窦多见,少数可为双侧多窦;(2)病变鼻窦多表现为密度不均匀的软组织影,密度相对较高,部分向鼻腔膨出;(3)多数病灶内可见沙粒样、小条片状、团块状钙化灶;(4)受累窦壁可见不同程度的骨质增生硬化,可伴有轻度骨质破坏,但残存骨端伴有骨质硬化。由毛霉菌感染引起4例:变应性真菌性鼻窦炎2例,CT表现为:鼻窦黏膜增厚;鼻窦腔内均匀高密度影,呈毛玻璃样特征,病变密度明显高于相邻软组织;可有斑点状钙化灶。另外可见窦壁骨质侵蚀破坏1例,病变侵犯眼眶1例,但骨质破坏较明显。结论真菌性鼻窦炎具有较为典型的CT表现,可准确地显示病变累及的范围,对诊断与治疗具有重要价值。

The CT Analysis of Fungal Sinusitis 25 Cases

Objective To discuss the CT imaging features of the fungal sinusitis.Methods The clinical data and CT imaging of 25 cases of fungal sinusitis patients were analyzed retrospectively.Results The 25 cases of fungal sinusitis,infections caused by Aspergillus 21 cases:(1)Sinus disease more than a single sinus to the maxillary sinus,most of the minority for the double multi-lateral sinus;(2) Sinus disease and more uneven performance for the higher density of the soft tissue shadow,the relatively high density,some visible to the nasal encephalocele;(3) The majority of lesions visible in the sand-like,small,flaky,clumps Calcification stove;(4) Involvement sinus wall that different levels of bone hyperplasia sclerosis,with mild bone destruction,but the remaining bone with bone-hardening;Hair fungal infection caused by the four cases,including performance for change Should be of two cases of fungal sinusitis,CT performance:Unilateral or bilateral group to pay the entire sinus involvement,sinus mucosal thickening,sinus cavity uniform high-density video,a ground-glass-like characteristics,disease density significantly higher than the adjacent soft tissue,A dot-calcified lesions;Performance for aggressive change in two cases,one involving the right maxillary sinus and ethmoid,one cases involving the left maxillary sinus,CT performance:Sinus disease in the uneven density increased soft tissue shadow,Relatively high density and bone erosion damage clearly visible,with mild bone sclerosis,around the PPS increased density,one case of visible to the same side of the orbit.Conclusion Fungal sinusitis have a typical CT findings,CT at the same time can be accurately show the scope of diseases involving,have the important value for the diagnosis and treatment.
