类风湿性关节炎患者外周血T淋巴细胞亚群分析

摘    要:
目的了解类风湿性关节炎患者外周血淋巴细胞亚群的分布特征,以探讨其发病机制。方法使用间接免疫荧光和直接免疫荧光方法对19例类风湿性关节炎患者和16例健康对照的外周血淋巴细胞进行研究。结果类风湿性关节炎患者与对照组比较CD3+T细胞和CD4+T细胞无明显变化,CD8+T细胞明显减少,CD4+/CD8+T比值增加。CD4+2H4+T细胞在CD4+T细胞中所占比例减少,而CD4+4B4+T细胞所占比例增加。进一步研究发现,类风性性关节炎患者活化的CD25+T淋巴细胞增加;IgG含量显著增加,IgM和IgA与正常组比较无明显差别。结论类风湿性关节炎患者存在细胞免疫和体液免疫的功能紊乱,而大量自身免疫球蛋白的产生可能是辅助T淋巴细胞升高,抑制性T淋巴细胞减少以及活化T淋巴细胞增加的结果,因此,T淋巴细胞亚群比例失衡在类风湿性关节炎的发病中可能起着更为重要的作用

Abstract:
Objective To explore the phenotypic characteristics of T lymphocytes in the peripheral blood (PB) of patients with rheumatoid arthritis (RA).Methods T lymphocytes in 19 patients with rheumatoid arthritis and 16 healthy controls were analyzed by using single and 2 color fluorescence flow cytometry.Result The percentages of CD3 cells and CD4 cells in PB from patients with RA and healthy controls were similar. but the percentage of CD8 cells was significantly lower (P<0.05) and the ratio of CD4 cells to CD8 cells was significantly higher (P<0.01) than that in PB from healthy controls. In addition, the percentage of CD25 cells was also increased compared to that in PB from healthy controls.The percentage of CD4 2H4 cells among CD4 cells in PB from patients with RA was markedly decreased (P<0.01), but the percentage of CD4 4B4 cells was significantly increased (P<0.01) compared with that in PB from healthy controls.Serum IgG concentrations in RA patients were significantly higher than in healthy controls. However, there were no significant differences between RA patients and healthy controls in the serum IgM and IgA.Conclusion A combination of the increment in helper T cells, the reduction in suppressor T cells, and the increment in activated T cells may lead to excessive production of autoantibodies in patients with RA.
