高频电波刀电圈切除术在宫颈疾病诊治中的应用价值

目的探讨高频电波刀电圈切除术（Loop electrosurgical excision procedure，LEEP）在宫颈疾病诊治中的临床应用价值。方法对286例经妇科检查、宫颈脱落细胞学检查、阴道镜及宫颈活检初步诊断为不同类型的宫颈疾病患者采用LEEP治疗。切除组织全部送病理检查。观察手术时间，术中、术后出血量，病人反应及术后创面愈合情况。结果LEEP治疗宫颈疾病一次成功率为95．1％（9．26％～97．6％），平均手术时间6．8分钟，出血量9．3毫升。无继发性感染发生，术后出血少。手术标本经病理学检查明确诊断，与术前相符者278例，病理升级者8例。结论LEEP治疗宫颈疾病操作简单、安全，成功率高，尤其适用于宫颈上皮内瘤变（CIN）。同时LEEP治疗可提供完整的病理标本，在治疗同时进行诊断。

Evaluation of loop electrosurgical excision procedure in the diagnosis and treatment of cervical diseases

Objective To evaluate loop electrosurgical excision procedure（LEEP）in the diagnosis and treatment of cervical diseases. Methods Different types of 286 patients with cervical diseases were treated with LEEP, all patients were primary diagnosised through gynecologic examination, cervical exfoliative cytological examination, colposcope and cervical biopsy. All excised tissues were delivered to pathological detection. Operative time, volume of bleeding, patient reaction and raw surface agglutination were observed. Results The effective rate was 95.1%. The mean operative time was 6.8 minutes and the volume of bleeding was 9. 3 ml. There was no secondary infection happened and less bleeding after the operation. Operative specimens were identified through pathological detection, which showed that 278 cases conformed to preoperative diagnosis, 8 cases showed upgrade of pathology. Conclusion The advantages to use the LEEP to management of the cervical diseases include its simpleness, safety and high cure rate.
