Clinical effect of modified posterior unilateral subtotal vertebral osteotomy in the treatment of old thoracolumbar fractures with kyphosis and its influence on complications

Objective To investigate the clinical effect of modified posterior unilateral subtotal vertebral osteotomy in the treatment of old thoracolumbar fractures with kyphosis and its influence on complications.Methods A retrospective selection of 100 patients with old thoracolumbar fractures with kyphosis admitted in our hospital from October 2015 to October 2019 were selected.According to the surgical method,the patients were divided into traditional posterior total vertebral resection group(traditional surgery group)and modified posterior unilateral subtotal vertebral osteotomy surgery group(modified surgery group),with 50 cases in each group.The surgery-related indicators,degree of low back pain,sagittal vertical axis(SVA),kyphotic Cobb angle,lumbar function and occurrence of complications were compared between the two groups.Results The intraoperative blood loss and autologous blood transfusion in the modified surgery group were significantly less than those in the traditional surgery group,and the operation time and bone healing time were significantly shorter than those in the traditional surgery group(P<0.05).After treatment,the VAS and ODI scores in the modified surgery group were significantly lower than those in the traditional surgery group,the SVA was significantly shorter than that in the traditional surgery group,and the kyphosis Cobb angle was significantly smaller than that in the traditional surgery group(P<0.05).The total incidence of postoperative complications in the modified surgery group was significantly lower than that in the traditional surgery group(P<0.05).Conclusion Modified posterior unilateral subtotal vertebral osteotomy in the treatment of old thoracolumbar fractures with kyphosis has a significant clinical effect,which can effectively reduce the incidence of complications.
