老年股骨粗隆间骨折固定方法选择

目的探讨老年股骨粗隆间骨折如何选择固定方法。方法依据老年患者股骨粗隆间骨折类型伴随的心、肺、脑疾患,选择不同的固定材料,共选取外固定支架、中空螺纹钉、动力髋螺钉及Gammar钉等固定材料,观察0.5～2年。结果中空螺纹钉内翻率15.4%,外固定支架内翻率33.3%,动力髋螺钉内翻率6.4%,Gammar钉内翻率5%。依照马元璋评定标准评定髋关节功能,中空螺纹钉固定组优良率88.4%,外固定支架固定组优良率75%,动力髋螺钉固定组优良率93.5%,Gammar钉固定组优良率95%。结论动力髋螺钉与Gammar钉为老年股骨粗隆间骨折固定的理想材料,依据老年患者身体情况,中空螺丝钉及外固定支架仍是股骨粗隆间骨折固定方法之一。

Choice of fixation method for aged thighbone carina space fracture

Objective To discuss how to choose the fixation method for aged thighbone carina space fracture.Method According to the types of thighbone carina space fracture and the concomitant heart,lung,brain disease,different fastness materials,including outside festness bracket,hollow screw,dynamical coxa screw and Gammar screw were chosen,the sufferers were observed for6-24months.Result Varus rate of hollow screw was15.4%;varus rate of outside fastness bracket,33.3%;varus rate of dynamical coxa screw,6.4%;varus rate of Gammar screw,5%.According to the assessment standard of Ma Yuanzhang:choiceness rate of hollow screw fastness was88.4%;of outside fastness bracket,75.0%;of dynamical coxa screw,93.5%,of Gammar screw,95.0%.Conclusion Dynamical coxa screw and Gammar screw are the ideal materials to fixation of aged thighbone carina space fracture;according to the condition of the sufferer,hollow screw and outside fastness bracket is still one the ways.
