99mTc-MIBI心肌灌注显像诊断小儿病毒性心肌炎的临床价值

摘    要:目的探讨和评价99mTc-MIBI心肌灌注显像诊断小儿病毒性心肌炎的临床价值.材料与方法73例临床诊断为病毒性心肌炎和58例非病毒性心肌炎患儿空腹静息下静脉注射111~740MBq99mTe-MIBI,60min后行常规体位平面/断层显像.结果73例临床确诊病毒性心肌炎患儿中有50例在不同体位的平面影像(36例)、断层影像(2例)和平面+断层影像(12例)有1~3节段呈花斑样改变,其敏感性为68.5%.非病毒性心肌炎组58例,8例有花斑样改变,假阳性率为13.8%,特异性86.2%,诊断准确率为76.3%.结论99mTc-MIBI心肌灌注显像不失为一种诊断小儿病毒性心肌炎的简便、无创、经济而又有较高的辅助诊断价值的检查方法.

Abstract:Purpose: To investigate the clinical value of myocardial perfusion imaging in the diagnosis of podiatric viral myocarditis. Materials and Methods: 73 cases with viral myocarditis and 58 cases without were studied with planar and/or tomographic imaging per formed 60 min after injection of 111 ~ 740MBq ~99mTc-MIBI. Results: 50 (planar imaging: 36 cases; tomography: 2 cases; planar to mography: 12 cases) of 73 cases of viral myocarditis showed "patchy" distribution of radioactivity in their myocardium. The sensitivity was 68. 5 %. 8 of 58 cases in nonmyocarditic group were false positive (13. 8 %). The specificity was 86. 2 %. The diagnostic accuracy was 76. 3 %. Conclusion: ~99mTc- MIBI myocardial perfusion imaging is proved a convenient, noninvasive and valuable approach for the diagnosis of pediatric viral myocarditis.
