克拉玛依市糖尿病健康教育模式及效果评价

目的探讨克拉玛依市糖尿病健康教育模式及效果评价。方法自2002年起连续3年对克拉玛依市社区人群进行糖尿病健康教育、糖尿病高危人群筛查,对筛查出的糖尿病患者与医院门诊、住院患者一起进行医院内糖尿病健康教育,对筛查出的糖调节异常患者与糖尿病高危人群一起继续接受社区糖尿病健康教育。结果2002~2004年克拉玛依社区糖尿病筛查结果提示克拉玛依地区糖尿病患病率无明显增加趋势(P>0.05)。通过3年的健康教育,社区人群的糖尿病相关知识、态度、行为均较前有明显提高。医院管理的糖尿病患者进行糖尿病相关检查的依从性逐年上升,糖化血红蛋白控制率逐年上升,2002年为26.00%,2003年28.00%,2004年为30.00%;未达标率逐年下降,2002年至2004年分别为:46.00%,43.00%,41.00%;尿微量白蛋白指数(A/C>30)2002年29.00%,2003年30.00%,2004年31.00%,各组比较均无差异性。3年来视网膜病变的检出率分别为63.00%,43.00%,42.10%,呈逐年下降趋势,有显著差异性。结论糖尿病健康教育对2型糖尿病患者及其高危人群健康知识的获得、依从性的增加和临床结果均有积极的影响,克拉玛依市社区和医院糖尿病健康教育模式已初见成效。

Health education mode of diabetes mellitus and its effectiveness evaluation in Karamay City

Objective To explore the health education mode of diabetes mellitus and evaluate the effectiveness in Karamay City.Methods From 2002,the diabetes mellitus health education and the screening for the persons inclined to be suffered from diabetes mellitus were carried out in the community in Karamay City.The diabetes mellitus health education was also performed for the patients with diabetes mellitus after the screening and the patients in clinic and inpatients.The patients of impaired glucose regulation(IGR) after the screening were educated together with the persons inclined to be suffered from diabetes mellitus.Results The screening result showed that the morbidity of diabetes mellitus in Karamay from 2002 to 2004 had not increased significantly(P>0.05.After three years of diabetes mellitus health education,the knowledge,attitude and behavior of the community residents in Karamay City had obviously been improved.The compliance of the patients of the hospital who initiated to have the diabetes detection had increased year by year,and the cortrol rate of HbA1C was 26.00% in 2002,28.00% in 2003 and 30.00% in 2004 respectively.The percent in diabetes mellitus control rised meanwhile the percent of no reaching the standard fell year by year,which was 46.00% in 2002,43.00% in 2003 and 41.00% in 2004.The index of microalbuminuria were,with A/C greater than 30,29.00% in 2002,30.00% in 2003 and 31.00% in 2004.The comparison of the two groups had no difference.It showed that the patients with diabetes mellitus neuropathy had not increased with the management in past three years.The detection rate of patients with retinal pathology was separately 63.00% in 2002,43.00% in 2003 and 42.10% in 2004,which showed that the rate decreased year by year,with significant difference.Conclusion The diabetes mellitus health education puts positive impact on the acquirement about healthy knowledge,the increasing of compliance and clinical results for the patients with diabetes mellitus type 2 and the persons inclined to suffered from diabetes.Diabetes mellitus health education mode in Karamay community and hospital was primarily successful.
