双重髌前“8”字张力带内固定治疗髌骨粉碎性骨折

目的：探讨双重髌前“8”字张力带治疗髌骨粉碎性骨折的方法与效果。方法对我院2009年1月～2012年6月收治的22例髌骨粉碎性骨折患者采用髌前纵行切口，以3枚平行克氏针纵向穿入大多数骨折块，再分别将相邻2枚克氏针经髌前“8”字钢丝捆绑，形成两组张力带。结果随访病例19例，随访时间9～24个月，骨折均达骨性愈合，髌骨关节面平整，6～12个月拆除内固定。功能依据Lysholm膝关节功能评分标准：优18例，良1例。结论双重髌前“8”字张力带治疗髌骨粉碎性骨折符合生物力学原理，固定牢靠，可早期进行膝关节功能锻炼，功能恢复佳。

Employment of double “8” figure tension band fixation in the treatment of comminuted patellar fractures

Objective To study the employment and outcome of double “8” figure tension band fixation in the treatment of comminuted patellar fractures .Methods Twenty-two cases of comminuted patellar fractures trea-ted in our hospital from Jan .2009 to Jun.2012 were treated with 3 Kirschner wires paralleling through the fractured bones after making anterior patellar longitudinal incision .Neighboring wires were fixed anterior the patella in the shape of “8” and formed two tension bands .Results Nineteen cases were followed up for 9-24 months.All pa-tients got bone union and their patellar articular surfaces were flat .The fixations were removed after 6-12 months. According to Lysholm knee scoring scale ,18 cases were excellent and 1 case was good .Conclusion Double “8”figure tension band fixation in the treatment of comminuted patellar fractures was firm and biomechanically accepta -ble,allowing early functional exercise and satisfactory function recovery .
