地佐辛用于妇科术后镇痛的临床观察

目的探讨地佐辛用于妇科术后患者自控静脉镇痛（PCIA）的临床效果及其安全性。方法 60例择期行开腹子宫切除术的患者随机分为两组,每组30例,D组予地佐辛0.6 mg/kg＋托烷司琼0.1 mg/kg加生理盐水至100 ml：F组予芬太尼15μg/kg＋托烷司琼0.1 mg/kg加生理盐水至100 ml,行PCIA治疗,分别记录术后4 h、8 h、12 h、24 h、48 h VAS疼痛评分及Ramsay镇静、不良反应发生情况,记录PCIA有效按压次数,患者总体满意度等。结果两组患者术后各时间点VAS评分和Ramsay镇静评分比较,差异无统计学意义（P〉0.05）,D组镇痛总体满意度明显高于F组（P〈0.05）,D组不良反应的发生率明显低于F组（P〈0.05）。结论与芬太尼相比,地佐辛用于妇科手术术后镇痛同样能提供满意效果,且不良反应发生率低。

Clinical observation of dezocine in patient-controlled intravenous analgesia underwent gynecological surgery

Objective To observe the analgesic effect and safety of patient-controlled intravenous analgesia with dezocine in patients underwent gynecological surgery.Methods 60 patients underwent elective hysterectomy were randomly divided into two groups,with 30 cases in each group.Group D(n=30): Dezocine 0.6mg/kg + tropistron 0.1 mg/kg + sodium chloride to 100 ml;group F(n=30): fentanyl 15 μg/kg+ tropistron 0.1 mg/kg + sodium chloride to 100 ml.The two groups background dose 2 ml/h,controlled dose 1.5 ml/time and lock time 15 minutes.The postoperative analgesic visual analogue scale and sedation scale at 4 h,8 h,12 h,24 h and 48 h were recorded.The effective PCA boluses,side effects as well as patients satisfactory degree were recorded and compared.Results Patients in the two groups at different time points,VAS scores and the Ramsay scores had no statistically significant difference.The overall patient satisfaction of group D were significantly higher than that of group F(P< 0.05),and the prevalence of side effects in group D was significantly lower than that in group F.Conclusion Compared with fentanyl,PCIA with dezocine may provide a satisfactory postoperative analgesia with less side effects.
