胎膜早破188例临床分析

目的 探讨胎膜早破的发病因素和母婴并发症以及临床处理策略.方法 对2010年2月～2011年2月期间188例胎膜早破的临床病例特点进行回顾性分析,并随机抽取同期正常分娩的200例作为对照组.结果 病因方面胎膜早破组感染、胎位异常、头盆不称、宫颈内口松驰、子宫畸形等明显高于对照组；妊娠结局方面胎膜早破组剖宫产、早产、产后出血、产褥感染、新生儿窒息、新生儿肺炎等明显高于对照组.结论 胎膜早破发病因素有自身特点,与难产互为因果,而且增加母要的并发症,对不同特点的胎膜早破应采取不同的治疗方法.

Clinical analysis on 188 cases of premature rupture of membranes

OBJECTIVE To investigate the incidence of premature rupture of membranes and maternal and fetal complications,and discuss the clinical treatment strategies.METHODS From February 2010 to February 2011,the features of 188 cases of premature rupture of membranes were analyzed retrospectively and randomly selected 200 cases with normal delivery as control group in the same period.RESULTS The etiologies like infection,abnormal fetal position,cephalopelvic disproportion,slack cervix,uterine malformations in the premature rupture of membranes group were significantly higher;pregnancy outcomes such as cesarean section,preterm labor,postpartum hemorrhage,puerperal infection,neonatal asphyxia,neonatal pneumonia in the premature rupture of membranes group were significantly higher.CONCLUSION The incidence of premature rupture of membranes has its own characteristics and interact as both cause and effect with difficult birth,and make maternal and child complications increase.It should take different approaches in treatment of premature rupture of membranes according to different characteristics.
