乙型肝炎病毒(HBV)耐药基因突变检测研究

摘    要:目的 建立HBV基因分型、耐药变异以及前C区/BC P区突变检测技术,为乙型病毒性肝炎诊断及监测提供依据.方法 选取某院2010年1月~2011年1月收治的54例乙型病毒性肝炎患者的血清样本,利用MassARRAY Assay 和PCR技术对样本进行耐药基因检测.结果 ① 54例患者中B基因型28例(51.85%);C基因型22例(40.74%);B,C混合基因型2例(3.70%);无D基因型;B,C,D以外的其他基因型1例(1.85%).②54例患者中,37例对拉米夫定耐药,37例患者对拉米夫定耐药的患者中,突变基因型均为B和C基因型.突变基因型主要是180M型(56.76%),无207I型,而且突变基因多见于C基因型,比例为72.97%,高于B基因型(27.03%),比较差异有统计学意义(P<0.05).结论 对拉米夫定耐药的患者中,HBV-DNA突变基因型主要是180M型,而且多见于C基因型,MassARRAYAssay技术可以准确检测HBV基因型及耐药基因突变位点,可以为临床诊断治疗提供依据.

Abstract:OBJECTIVE To establish HBV genotyped,drug-resistant mutant and former C area /BC P area gene mutation testing technology,and provide basis for diagnosis and clinical monitoring of HBV.METHODS From January 2010 to January 2011,the serum sample of 54 patients with hepatitis B virus Hepatitis were selected to test drug-resistant genetic by MassARRAY Assay and PCR technology.RESULTS ① In 54 patients,28 cases were B genotype(51.85%);22 cases were C genotype(40.74%);2 cases were B,C mixed genotype(3.70%);there was no D genotype;other genotype was 1 case(1.85%).② 37 cases of patients were lamivudine resistance in 54 patients,and the mutated gene types were B and C genotype.The main mutated gene was 180M type(56.76%),and there was no 207I type.The main mutated genetypo was C genotype,the proportion was 72.97%,which was higher than B genotype(27.03%),the difference was statistically significant(P﹤0.05).CONCLUSION In lamivudine resistance,the HBV-DNA mutated gene is 180M genotype,and more common in C genotype.MassARRAY Assay technology can accurately detect HBV genotype and drug-resistant gene mutations,and provide the basis for clinical diagnosis and treatment.
