切挂选择性缝合术治疗高位肛瘘的临床研究

目的观察切挂选择性缝合术治疗高位肛瘘临床有效性,评估术后肛门功能恢复情况。方法按高位肛瘘诊断标准及排除标准纳入病例60例,治疗组30例采用切开挂线、旷置、缝合相结合术式,对照组30例采用切挂旷置术。结果经术后6个月随访,治疗组30例中治愈26例,治愈率86.67%;1例出现污染内裤,平均病程(23.27±4.13)d,并发症发生率3.3%。对照组30例中治愈25例,治愈率83.33%。6例出现污染内裤,平均疗程(25.39±5.21)d,并发症发生率20%。结论与对照组相比,切挂选择性缝合术对治疗高位肛瘘具有治愈率高、痛苦小、疗程短、并发症少、术后肛门功能损伤小等优点。

Clinical study on incision cutting section drainage and selective suture in the treatment of high anal fistula

Objective To evaluate the efficiency of incision cutting section drainage and selective suture in the treatment of high anal fistula,and anal function post-operation.Methods 60 cases were randomly divided into treatment group(n=30)and control group(n=30).The operation of the treatment group were incision cutting section drainage,lay-open and selective suture,and the operation of the control group were incision cutting section drainage and lay-open.Wexner score,operation time,curve time,anorectal manometry parameters were observed.Results After 6 months following-up,the cure rate of the treatment group was 86.67%,only one case recurred,the mean disease course(23.27±4.13)day,with the incidences of complications was 3.3%;the cure rate of the control group was 83.33%,six cases recurred,the mean disease course(25.39±5.21)day,with the incidences of complications was 20%.Conclusion Comparing with control group,it has advantages of high cure rate,less pain,shortening treatment course,less complications and less injury of anal function in the treatment group.
