179例过敏性紫癜临床及实验室分析

目的探讨过敏性紫癜(Henoch-Schonlein purpura,HSP)的临床及实验室检查特征。方法回顾分析2006年1月～2011年6月诊断明确、在我科住院治疗的179例HSP患者的临床及实验室检查资料。数据采用Epi data 3.0进行数据录入,SPSS 11.5进行统计分析。结果所有HSP患者的男女性别比为1∶0.74,所有患者中以感染作为可能诱因有114例,比例最大,占63.7%。成人组与儿童组诱因比较无统计学差异。首发症状最为常见的是下肢紫癜,其次为关节痛及腹痛。儿童患者紫癜发生于上肢及头面部几率高于成人。关节痛以踝关节最易受累,儿童组比成人组更易累及关节。成人组中尿蛋白发生几率高于儿童组。结论 HSP在成人及儿童中均可以患病,感染在成人和儿童患者中都是疾病的危险诱因,成人患者更易发生肾脏受累,在随访中需要更加严密观察。

Analysis of clinical and laboratory data in 179 patients with Henoch-Schonlein purpura

Objective To explore the clinical manifestations and laboratory findings of Henoch-Schonlein purpura. Methods The clinical features and laboratory data of 179 cases of HSP were retrospectively analyzed with SPSS 11.5 statistical software. Results The data of all of 179 patients with HSP were collected, including 103 men and 79 women cases with a ratio of male to female of 1：0.74. The major potential causes of HSP were infection in 114 cases （63.7%）. There was no significant difference in the inducing factors between adults and children. The common symptoms of HSP were ecchymosis of lower limbs, arthralgia and abdominal pain. The rate of ecchymo- sis distributed on upper limbs and face in children was higher. Arthralgia occurred more often in children, but albuminuria occurred more often in adults. Conclusion Infection may be one of the risk factors of HSP development in adults and children. The damage of kidney function occurred more often in adults with HSP, and we should pay more attention to it during follow-up.
