摘 要: | 目的 探讨非乙醇性脂肪肝病人门静脉、肝静脉管径及血流频谱的变化特点。方法 运用彩色多普勒超声检测159例轻、中、重度非乙醇性脂肪肝病人门静脉、肝静脉管径及血流频谱,与40例正常肝组织作对比分析。结果 轻度脂肪肝各项检查指标与正常组比较无明显差异,随脂肪肝程度加重,肝静脉管径逐渐变小、异常波形增多,门静脉管径逐渐增大、血流速度减慢;轻、中、重度脂肪肝肝静脉管径与异常血流频谱(平坦波和衰减波)、门静脉管径与门静脉血流速度各组间比较差异有显著性(t=2.66~3.85;χ^2=10.77、5.53,P〈0.05)。结论 非乙醇性脂肪肝肝静脉、门静脉管径及血流频谱的变化有其自身特点,且与病情的严重程度相关,多普勒血流频谱变化可能成为病情严重程度评估的客观指标之一。

Abstract: | Objective To investigate the caliber of portal and hepatic vein in non-alcoholic fatty liver and its features showed on doppler waveform. Methods The calibers of portal and hepatic veins of 159 patients with mild, moderate and severe non-alcoholic fatty liver were detected by Doppler ultrasonography and compared with 40 controls. Results There was no significant difference between mild cases and the controls. With the aggravation of the disease, the hepatic vein became narrowed, abnormal waveform increased, the portal vein widened, and blood flow slowed down. The differences between mild, moderate and severe groups were significant in terms of the caliber of hepativ vein and abnormal waveform as well as the portal vein and its blood flow (t=2.66-3.85;Х^2=10.77,5.53;P〈0.05). Conclusion In fatty liver, the changes of the caliber and doppler waveform of portal and hepatic vein have their respective characters, which is associated with the severity of the disease. The Doppler flow changes may become one of the objective parameters for evaluation of the severity of this illness. |
