二维超声心动图与左室造影用于曲美他嗪治疗冠心病左心功能不全疗效评价

目的探讨二维超声心动图与左室造影在评价曲美他嗪治疗冠状动脉粥样硬化心脏病(简称冠心病)左心功能不全疗效中的应用效果。方法选取石家庄市第一医院2017年11月至2018年11月用曲美他嗪治疗的冠心病左心功能不全患者80例,分别应用二维超声心动图与左室造影检查并评价疗效。结果80例患者中显效27例,有效45例,无效8例,总有效率为90.00%。二维超声心动图测得患者治疗后的E/A值和左心室射血分数(LVEF)显著高于治疗前,左室舒张末期容积(LVEDV)和左室收缩末期容积(LVESV)显著低于治疗前(P<0.05);左室造影测得患者治疗后的心排出量(CO)、心脏指数(CI)和每搏输出量(SV)显著高于治疗前(P<0.05)。患者治疗后的每周心绞痛发病次数、N末端B型脑钠肽(NT-proBNP)水平均显著低于治疗前,6 min步行距离(6MWT)显著长于治疗前(P<0.05);患者治疗过程中均未出现明显不良反应。结论应用曲美他嗪治疗冠心病左心功能不全有一定疗效,且应用二维超声心动图与左室造影检测心功能准确性较高。

Two-Dimensional Echocardiography and Left Ventricular Angiography in Evaluating the Efficacy of Trimetazidine in the Treatment of Left Ventricular Dysfunction in Patients with Coronary Heart Disease

Objective To explore the application of two-dimensional echocardiography and left ventricular angiography in evaluating the efficacy of trimetazidine in the treatment of left ventricular dysfunction in patients with coronary heart disease.Methods Eighty patients with left ventricular dysfunction of coronary heart disease treated with trimetazidine from November 2017 to November 2018 were selected.The therapeutic effects were evaluated by two-dimensional echocardiography and left ventricular angiography.Results Among the80 patients,27 were markedly effective,45 effective and 8 ineffective.The total effective rate was 90.00%.The E/A value and left ventricular ejection fraction(LVEF)of the 80 patients after treatment by two-dimensional echocardiography were significantly higher than those before treatment,and the left ventricular end-diastolic volume(LVEDV)and left ventricular end-systolic volume(LVESV)were significantly lower than those before treatment(P<0.05).The levels of cardiac output(CO),cardiac index(CI)and stroke volume(SV)in the 80 patients with left ventricular angiography after treatment were significantly higher than those before treatment(P<0.05).The incidence of angina pectoris and the level of NT-proBNP in the 80 patients per week after treatment were significantly lower than those before treatment;6-minute walk test(6 MWT)was significantly longer than that before treatment,and the difference was statistically significant(P<0.05).There were no adverse reactions during the treatment.Conclusion Trimetazidine is effective in the treatment of left ventricular dysfunction in patients with coronary heart disease,and the accuracy of two-dimensional echocardiography and left ventricular angiography is higher.
