神经内窥镜手术治疗梗阻性脑积水

目的:介绍神经内窥镜技术治疗梗阻性脑积水,分析内窥镜技术的手术技巧和并发症的预防。方法:施行内窥镜手术47例,针对不同病因导致的梗阻性脑积水分别施行三脑室底造瘘术、透明隔造瘘术、室间孔成形术和囊肿脑室造瘘术等等。结果:临床有效率为83%,术中出血4例(8.5%),死亡1例(2.1%)。结论:神经内窥镜已成为梗阻性脑积水的首选治疗方案,但应严格掌握手术适应证和有效预防并发症。

Treatment of Obstructive Hydrocephalus with Neuroendoscopy

Objective: To treat obstructive hydrocephaluses caused by different reasons with neuroendoscopy, and to study operative skills and precaution of complications in neuroendoscopic operations. Methods: 47 patients were treated, in which 25 cases suffered from the obstruction of the Sylvian's tube, 5 cases from colloid cysts in the third ventricle, 3 cases from arachnoid cysts in the third ventricle, 2 cases from brain cysticercosis in the third ventricle, 10 cases from single obstruction of Moroe's hole, and 2 cases from obstruction of both Moroe's holes. They were operated with the third ventriculostomy, cyst-ventriculostomy, resection of cyst, and septum pellucidumostomy, respectively. Results: The clinical effective rate of all cases was 83 percent. 4 cases suffered from intraoperative bleeding, 1 cases died. Conclusion: Neuroendoscopic technique has been the primary method for the obstructive hydrocephalus.
