彩色多普勒超声和超声造影诊断小肝癌的临床应用价值

摘    要:
目的:探讨彩色多普勒超声和超声造影诊断小肝癌的临床价值及意义。方法收集经病理证实的小肝癌患者30例,分析其超声影像资料。结果 CDFI显示病变周边Vmax(80.30±9.10)cm/s,RI(0.70±0.11),28个(75.68%)可探及动脉频谱;病变内部Vmax(58.17±8.24)cm/s,RI(0.72±0.13),32个(86.49%)可探及动脉频谱。超声造影显示病灶动脉相均呈高增强,动脉中晚期或门脉早期快速廓清,呈快进快退表象。不同分化程度小肝癌的超声造影增强时间及达峰时间差异无统计学意义(P跃0.05),而分化程度高者的开始廓清时间明显晚于分化程度低者(P〈0.05)。结论小肝癌CDFI和超声造影具有特殊的征象,超声造影并能初步评价其病理分化程度,可以提高正确诊断率,因此超声造影诊断小肝癌具有良好的临床应用前景。

Abstract:
Objective To investigate the clinical value and significance of color doppler ultrasonoscope (CDFI) and con-trast-enhanced ultrasound in the diagnosis of small hepatocellular carcinoma. Methods Thirty cases patients with small hepatocellular carcinoma were collected, which were confirmed by pathology, and ultrasonic audio-visual mate rials were analyzed. Results CDFI showed lesions surrounding Vmax (80.30±9.10) cm/s, RI (0.70±0.11), 28 lesions (75.68%) could be detected arterial spectrum; internal lesions Vmax (58.17±8.24) cm/s, RI (0.72±0.13), 32 lesions (86.49%) could be detected arterial spectrum. The contrast-enhanced ultrasound characteristics of small hepatocellular carcino-ma were rapid enhancement in early artery period, rapid clearance in late artery period or early portal period, that were characterized by fast enhancement and clearance.The differences of contrast-enhanced ultrasound enhancement time and peak time in different pathological differentiation of small hepatocellular carcinoma were no statistically significant (P〈0.05), but the differences of time to clarify were statistically significant (P〈0.05). Conclusion CDFI and contrast-enhanced ultrasound of small hepatocellular carcinoma with special signs, contrast-enhanced ultrasound can prelimi-nary evaluate the pathological degree of differentiation and improve the diagnostic correct rate, it is valuable for further clinical application.
