脑底动脉观测及其在介入放射学中的意义

摘    要:
目的:探讨脑底动脉环各组成血管的起源、分支、走行方向及其变异,为神经介入性插管提供解剖学依据。方法:对60例福尔马林同定的头颅标本,获取Willis环并充分暴露脑底部血管后,观测脑底动脉环各组成血管的起源、分支、走向、变异及其长度和外径。结果:虽然脑底动脉变异较大,但仍有规律可循,81.5%的ACA起源于ICA的前壁;67.2%的ACoA连于ACA的内侧壁;75.5%的PCoA起源于ICA的后外侧壁;88.3%的PCA起源于BA的外侧壁等。结论:组成脑底动脉环各动脉的长度及直径的推荐值,与临床介入性插管有关的脑底动脉的开口方位,可作为介入神经影像学在临床介入性插管和预制特定形态的导管、导丝时的重要参考.

Abstract:
Objective: To explore the origin, branch, course and variation of the arteries on the base of the brain and provide the anatomical basis for interventional neuroradiologic procedures. Methods: 60 cases chinese adult cadaveric brain were well prepared for the sake of surveying the origin, branch, course and variation of circle of willis and measuring its length, external diameter. Results: Regardless of the fact that variations of arteria basilaris were very common, laws can be also followed during the interventional approach: 81.5% ACAs originated from the anterior wall of ICAs, 67.2% ACoAs communicated with the lateral wall of ACAs, 75.5% PCoAs came from the posterior-external wall of ICAs and 88.3% PCAs came from external wall of BAs. Conclusions: The consulting length and diameter data of circle of Willis provided for chinese are offered in this study. Those are useful for catheterization and beneficial to prefabricate special catheters and guding wires.
