四肢关节结核的X线及CT诊断

目的:探讨四肢关节结核的X线平片和CT表现,评价X线平片和CT在四肢关节结核的应用价值。材料与方法:对经手术或临床证实的24例四肢关节结核的影像学资料进行回顾性分析。所有患者均行X线平片检查,15例同时行CT扫描。结果:23例为单关节受累,1例为两个关节受累。8例发生在膝关节,7例发生在髋关节,3例发生在踝关节,2例发生在腕关节,2例发生在肘关节,发生在跖趾关节、肩关节各1例。表现为骨质破坏15例,表现为骨质疏松7例,仅表现为软组织肿胀而无骨异常改变者2例结论:X线平片在四肢关节结核中可作为初步筛选方法:,CT能提供更多的诊断信息,两者在四肢关节结核的诊断中起着不可或缺的作用。

Imaging Diagnosis of Joint Tuberculosis of the Appendicular Skeleton

Purpose:To analyse the imaging appearance of X-ray and CT jointtuberculosis of the appendicular skeleton. Materials and Methods:Total 24 cases of joint tuberculosis of the appendicular skeleton proved by operation or clinical diagnosis were reviewed retrospectivelv,all have been taken X-ray,15 of them were underwent CT together.Results:23 cases of joint tuberculosis happened to one joint,and 1 case happened to two joints.The lesions localized at knee joint in 8 cases,hip joint 7 cases,ankle bone in 3 cases,wrist joint in 2 cases,metatarsophalangeal joint in 2 cases,elbow jo-int in 1 case,and shoulder joint in 1 case.15 cases were showed destruction of bone.7 cases were appeared osteoporosis.2 cases were only showed swelling in soft tissue without any abnormal appearance in bone.Conclusion:X-ray film can be choosed as preliminary screening of the lesion.CT can offer more information in diagnosis.CT and X-ray play an indispensable role in diagnosis of osteoarticular tuberculos-is.
