摘    要:
目的评价经皮激光椎间盘气化术(PLDD)联合臭氧(O3)髓核氧化术治疗腰椎间盘突出症的疗效。方法在CT引导下采用16G、21G穿刺针经皮穿刺至椎间盘髓核区,由穿刺针插入600μm光导纤维,用自动脉冲式激光切割、汽化髓核,以每个椎间盘500~1000 J切割、汽化,再向盘内注入浓度60 mg/L O3约10~20 m l,共治疗102例200节椎间盘。结果 本组穿刺成功率为100%,术中无严重并发症发生。随访12~24个月后,102例中,70例疗效为优(69%),26例为良(25%),4例为可(4%),2例为差(2%)。结论 CT引导下经皮PLDD联合O3治疗腰椎间盘突出症疗效佳、创伤小、痛苦少且安全,值得临床推广。

Abstract:
Objective To explore the therapeutic effects of percutaneous laser disc decompression(PLDD) combined with ozone nucleus gelatinous oxidizing technique to treat lumbar intervertebral disc protrusion(LIDP).Methods Under CT guidance,the 56-and 21-gauge the spinal needles were inserted into the space of the nucleus gelatinous,and then 600μm optical fibre was placed in.All 102 patients with 200 intervertebral discs were treated by auto pulsed-wave mode laser cutting and vaporizing(with the power of 500-1000 J for each intervertebral disc) and then O3(10-20 ml with concentration of 60 mg/L) was influxed.Therapeutic effects were judged by Macnab's standard.Results Among the total 102 patients with follow up of 12 to 24 months,70(69%) ones gained excellent therapeutic effects,26 ones(25%) gained good therapeutic effects,4 ones(4%) gained so-so therapeutic effects,and 2 ones(2%) gained no therapeutic effects.Conclusion PLDD combined with ozone nucleus gelatinous oxidizing technique is a safe and effective treatment method to LIDP.
