血沉、C反应蛋白及ECT联合诊断假体周围感染临床研究

目的：探讨联合检测血沉、C反应蛋白及ECT全身骨扫描对假体周围感染的诊断价值。方法：回顾性分析57例人工关节翻修患者临床资料，以术前血沉、C反应蛋白及ECT结果进行诊断，其中任2项阳性即诊断为假体周围感染，任2项阴性即诊断为无菌性松动，其结果与“金标准”进行对比分析。结果：血沉灵敏性、特异性及准确性分别为64．76％，86．96％和73．68％，C反应蛋白分别为70．59％，91．30％和78．95％，ECT分别为52．94％，82．60％和64．91％，3种方法联合检测时其结果分别为97．06％，95．65％和96．49％。结论：联合检测血沉、C反应蛋白及ECT骨扫描可提高假体周围感染的诊断。

Clinical study on erythrocyte sedimentation rate, C-reactive protein and ECT for periprosthetic infection

Objective To explore the clinical value of the combination of erythrocyte sedimentation rate, C-reactive protein and ECT bone scan to the detection of periprosthetic infection. Methods The clinical data were retrospectively analyzed in 57 patients with revision operation. The results of erythrocyte sedimentation rate, C-reactive protein and ECT bone scan were used as criteria to diagnose periprothetic infection. When any two results of the above examinations were positive, the patient was diagnosed with periprosthetic infection. When any two results of the above examinations were negative, the patient was diagnosed with aseptic loosening. The diagnostic results were compared with the ＂gold standard＂. Results The diagnostic sensitivity, specificity and accurate value in assessing periprothetic infection were 64. 76%, 86. 96% and 73. 68% with erythrocyte sedimentation rate, 70.59%, 91.30% and 78. 95% with C-reactive protein, 52. 94%, 82. 60% and 64. 91% with ECT, and were 97. 06%, 95. 65% and 96. 49% with the combination of three methods. Conclusion The combination of rythrocyte sedimentation rate, C reactive protein and ECT may improve the diagnosis of periprothetic infection.
