3种椎管内麻醉在子宫全切术中的比较

目的观察双间隙腰硬联合麻醉在子宫全切术中的应用效果。方法选择择期行子宫全切术患者90例,随机分成3组:Ⅰ组采用单间隙硬膜外麻醉,Ⅱ组采用单间隙腰硬联合麻醉,Ⅲ组采用双间隙腰硬联合麻醉,记录麻醉过程中的相关参数、麻醉效果、术后并发症和术后止痛的满意度。结果3组间循环系统指标比较无显著性差异;Ⅱ组、Ⅲ组的麻醉优良率明显高于Ⅰ组(P<0.01),以Ⅲ组为最优,术后止痛以Ⅲ组为最佳。结论双间隙腰硬联合麻醉用于子宫全切术既可获得完善的术中、术后镇痛,又可为手术提供良好的肌松效果。

Comparison of the effects of three kinds of intraspinal block anesthesia in total hysterectomy

Objective It is to observe the effect of double-space spinal-epidural block anesthesia in total hysterectomy.Methods 90 patients going to accept selective operation of total hysterectomy were chosen and randomly divided into three groups: group Ⅰ with single space epidural block anesthesia,group Ⅱ with single space spinal-epidural block anesthesia,group Ⅲ with double-space spinal-epidural block anesthesia.Correlated parameter during anesthesia,anesthesia effect,postoperative complications and satisfactory degree of anesthesia were recorded.Results Compared with each other,there was no significant difference in the indexes of circulatory system in the three groups.Good anesthesia rates in group Ⅱ and group Ⅲ were obviously higher than that in group Ⅰ(P<0.01),and that in group Ⅲ was the best.Postoperative anesthesia effect in group Ⅲ was the best too.Conclusion Double-space spinal-epidural block anesthesia has a satisfactory anesthesia effect during and after total hysterectomy,with a good flaccidity effect on the operation.
