腹腔注射左旋精氨酸诱导急性坏死性胰腺炎大鼠肺损伤的实验研究

摘    要:
目的 探讨大剂量左旋精氨酸腹腔注射诱导大鼠急性坏死性胰腺炎相关性肺损伤模型。方法将48只sD大鼠随机分成对照组(c组,n=24)、急性胰腺炎组(A组,n=24)。A组用6%的左旋精氨酸(L-Arginine,L-Arg)腹腔内注射,每次1.5mg/g体重,共3次,注射间隔1h,诱导ANP和APAu模型;c组同法注射等量生理盐水。在注射完L-Arg后的第6、12和24小时三个时点分批处死大鼠,分别测定两组各时点血清淀粉酶水平,并观察各组对应各时点胰腺和肺组织病理学改变的变化。结果A组大鼠胰腺和肺组织病理损伤在各时点比C组明显加重(P〈0.01);在肺损伤评分均呈正相关(rs=0.374,P=0.046;rs=0.452,P=0.027)。结论大剂量L-精氨酸分次腹腔内注射可成功诱导急性坏死性胰腺炎相关性肺损伤大鼠模型,ANP肺组织损伤程度与胰腺组织损伤程度密切相关。

Experinental study on pulmonary injury in severe acute nectotizing pancreatifis induced by L-arginine in rats

Abstract:
Objective To establish the model of acute nectotizing panereatitis (ANP) -associated lung injury induced by L-arginine in rats. Methods Forty-eight Spraque-Dawley male rats were randomly divided into tow groups C and A. ANP and APALI were induced by injected intraperitoneally three times with 1.5 mg/g body weight 6% L-Arg in an interval of 1 hour in group A. Group C received saline alone on the same methods. Rats were killed at the 6th,12th and 24th hour after the last time of L-Arg injection. The pathological changes of tissue of pancreatic and lung were respectively analyzed and scored according to Kusser' s and Lei weizhang' s standard. Results With time going,the pathological injuries of the pancreas and lung in group A were significantly more severely than that in group C(P〈0. 01). The serum amylase in group A was significantly higher than that in group C,especially at the 24th hour(P 〈0.01 ). Pathological injury score in pancreatic showed a correlation with the level of amylase and the lung injury (rs = 0. 374, P = 0. 046 ; rs = 0. 452,P = 0. 027). Conclusion The rat model of acute nectotizing pancreafitis (ANT) -associated lung injury can be induced by intraperitoneal injection of L-arginine( 1.5 mg/g,three times) at the concentration of 60g/L at an interval of 1 hour. Lung injury is closely related to pancreatic tissue injury in ANP.
