平喘化痰汤联合肺康复治疗卒中肺部感染后气管切开临床观察

目的观察平喘化痰汤联合肺康复治疗卒中肺部感染后气管切开临床疗效。方法将80例卒中肺部感染后气管切开患者按照随机数字表法分为2组。对照组40例予西医常规治疗;观察组40例在对照组治疗基础上加用平喘化痰汤联合肺康复治疗。2组均治疗14 d后统计临床疗效,并观察2组治疗前后咳嗽咯痰、呼吸困难评分,血氧饱和度、最大吸气量及最大呼气量情况。结果观察组总有效率92.5%(37/40),对照组总有效率77.5%(31/40),观察组临床疗效优于对照组(P<0.05)。治疗后2组咳嗽咯痰、呼吸困难评分均降低(P<0.05),且观察组低于对照组(P<0.05)。治疗后2组血氧饱和度均升高(P<0.05),且观察组高于对照组(P<0.05)。治疗后2组最大吸气量、最大呼气量均升高(P<0.05),且观察组高于对照组(P<0.05)。结论平喘化痰汤联合肺康复治疗卒中肺部感染后气管切开疗效确切,能改善临床症状、血氧饱和度、最大吸气量和最大呼气量。

Clinical observation of Pingchuan Huatan decoction combined with pulmonary rehabilitation in the treatment of pulmonary infection caused by tracheotomy after stroke

Objective To observe the effect of Pingchuan Huatan decoction combined with pulmonary rehabilitation in the treatment of pulmonary infection caused by tracheotomy after stroke.Methods 80 patients with pulmonary infection after tracheotomy were divided into two groups according to the random number table method.40 cases in control group were treated by conventional western medicine.40 cases in observation group were treated by Pingchuan Huatan decoction combined with pulmonary rehabilitation on the basis of the control group.After 14 days of treatment,the clinical effects of the two groups were statistically analyzed,and the scores of cough,expectoration,dyspnea,blood oxygen saturation,maximum inspiratory capacity and maximum expiratory capacity were observed before and after treatment.Results The total effective rate of the observation group was 92.5%(37/40),and that of the control group was 77.5%(31/40).The clinical effect of the observation group was better than that of the control group(P<0.05).After treatment,the scores of cough,expectoration and dyspnea in the two groups were lower than those in the group before treatment(P<0.05),and the observation group was lower than the control group(P<0.05).After treatment,the blood oxygen saturation of the two groups was increased(P<0.05),and the observation group was higher than the control group(P<0.05).After treatment,the maximum inspiratory capacity and expiratory capacity of the two groups were increased(P<0.05),and the observation group was higher than the control group(P<0.05).Conclusion Pingchuan Huatan decoction combined with pulmonary rehabilitation is effective in the treatment of pulmonary infection caused by tracheotomy after stroke,which can improve the clinical symptoms,the maximum inspiratory capacity and the maximum expiratory capacity of blood oxygen saturation.
