软组织肉瘤局部切除加组织间照射治疗──附10例报告

１０例侵犯主要血管、神经和骨的肢体局部晚期软组织肉瘤，经局部切除加后装放射治疗并随访６～２４个月，除１例因肺、脑转移死亡，１例肺转移带瘤生存外，余８例均无局部复发或远处转移，仅１例切口延迟愈合。作者介绍了瘤床置管、后装（１９２）铱放射治疗的方法和注意事项，认为该法可有效控制局部晚期软组织肉瘤复发，并能较满意保留肢体功能。

Local Resection and Brachytherapy in the Treatment of Soft Tissue Sarcoma of the Limb

Local resection plus interstitial radiation(brachytherapy)was used in 10 patients with local advanced soft tissue sarcoma in the lower extremity where tumor has involved adjacent neurovascular bundle or bone.The brachytherapy was carried out following the resction using iridium-192 for a total tissue dose of 40Gy in 5 days.Thence an external irradiation was added by 6 MeV linear accelerator for 30Gy.Follow-up at 6-24 months disclosed that 8 cases were alive without recur rence or metastasis except for one patient who died of lung and brain metastasis at 5 months and another one who survived with lung metastasis at 12 months.The wound healing was delayed for 3 months in one case.The results indicate that the combined function-saving excision and brachytherapy could control local recurrence with preservation of the limb function .By this way many amputations could be avoided.
