PDCA循环在全膝关节置换术后患者患肢肿胀护理中的应用

目的探讨PDCA循环在全膝关节置换术后患者患肢肿胀护理中的应用效果。方法 2013年3月至2014年5月,便利抽样法选取在无锡市第三人民医院骨科住院行全膝关节置换术后肢体肿胀的60例患者为研究对象,分析其肢体肿胀情况。依据PDCA循环进行持续质量改进,评估患者肢体肿胀控制的效果。结果实施PDCA循环后,患者术后72h内的疼痛评分均≤5分,其中:术后24h平均疼痛评分为2.8分,术后48h为3.8分,术后72h为3.9分,术后第4天为2.6分;护士对术后肢体肿胀知识的有效掌握率由培训前的74%增至96%;患者对全膝关节置换术护理总体满意度为92%。结论对全膝关节置换术后患者患肢肿胀度管理进行持续质量改进,提高了护士对术后肢体肿胀知识的有效掌握率,减轻了患者术后肢体肿胀度从而降低疼痛强度,提高了患者对护理的满意度。

Application of PDCA Circle in Limb Swelling Care of Patients Underwent Total Knee Arthro-plasty

Objective To improve the application effect of PDCA circle in limb swelling care of patients underwent total knee arthroplasty.Methods By convenience sampling,60 cases were selected,and their limb swelling situation was analyzed.And implement the continuous quality improvement according to PDCA circle.The control of swelling was evaluated.Results After the implementation of CQI,the most severe pain score within 72 hours after total knee arthroplasty in 60 patients were all lower than 5 points. The average pain score was 2.8 points within 24 hours,3.8 points within 48 hours,3.9 points within 72 hours,2.6 points the first four days after surgery.After training,the knowledge that the nurses can master of limb swelling increased from 74% to 96%,the patient satisfaction rate was 92%.Conclusion The implementation of continuous quality improvement in limb swelling care of total knee arthroplasty can effectively improve the mastery of knowledge of nurses,reduce postoperative limb swelling thereby reduce the pain of patients and increase patient satisfaction.
