腰腿痛胶囊对大鼠自体髓核硬膜外移植神经根功能的影响

摘    要:为探讨椎间盘源性腰腿痛的病理机制及腰腿痛胶囊对自体髓核硬膜外移植大鼠神经根功能恢复的影响,将48只健康、雄性大鼠,随机分成假手术组(SG)、对照组(MG)、腰腿痛胶囊低剂量组(LG)、腰腿痛胶囊高剂量组(HG组),每组12只.在麻醉下,各组动物距肛门1cm处截断尾骨,SC组仅咬除右侧半椎板,显露L5神经根;其余各组皆取自体尾椎髓核(约10mg)植入右侧L5神经根背侧.分别在术后第7、14、28天检测大鼠马尾神经MEP、SEP.结果显示,SG组手术前后诱发电位的改变无统计学意义;MG组术后波幅降低,潜伏期延长,术后14天波幅下降最低,潜伏期延长最明显;LG组术后7和14天、HG组术后7天波幅下降,潜伏期延长,与自身术前比较差异显著(P<0.05或P<0.01);LG组移植术后28天、HG组14天马尾神经诱发电位开始恢复.表明在无明显机械压迫情况下,大鼠自体尾椎髓核移植硬膜外腔造成了脊神经根结构与功能损害,腰腿痛胶囊可减轻或延缓该损伤.可能是通过降低神经根局部组织的炎性介质和(或)自身免疫反应而起作用.

Abstract:The purpose of the study is to explore the pathological mechanism of related-to-intervertebral-disk lumbocrural pains and to study the influence of Capsules for Lumbago and Scelalgia (CLS) on the rat nerve-root function after epidural transplantation of autogenous nucleus pulposus. Forty-eight healthy male rats were randomly divided into four groups: the shame-operation (SG), the model-control (MG), the low-dose-CLS-treated (LG) and high-dose-CLS-treated (HG) (n=12 each). Except for SG receiving only hemi-laminectomy and L5 nerve root exposure, the rest three groups received epidural transplantation of the autogenous coccygeal nucleus pulposus (10mg) to the dorsal L5 nerve root; the MEP and SEP of the cauda equina nerve root were detected 7, 14 and 28 days after operation, respectively. The results showed: (1) evoked potential change having no statistical significance in SG before and after operation; (2) an amplitude descent and latent period lengthening in MG after operation, which reached the greatest 14 days after operation; (3) an amplitude descent and latent period lengthening 7 and 14 days after operation in LG and 7 days after operation in HG, which had a significant difference (P < 0.05 or P < 0.01) compared with themselves before operation; (4) the evoked potentials began to recover 28 days and 14 days after the transplantation in LG and in HG, respectively. From the results, it can be seen that under no significant mechanical compression, CLS could remit and delay the injury to the structure and function of the spinal nerve root that results from epidural transplantation of the autogenous coccygeal nucleus pulposus; CLS may take an effect through reducing the inflammatory medium of the local nerve-root tissues or the autoimmune reaction.
