经直肠超声引导择点穿刺前列腺活检的应用价值

目的探讨经直肠超声(TRUS)引导择点穿刺前列腺活检在临床的应用价值。方法首先对139例疑诊前列腺癌(PCA)的患者行经直肠超声对前列腺作详细的观察,对发现异常病灶区确定为穿刺点,同时确定穿刺的针数。然后在超声引导下进行择点穿刺前列腺。结果 139例患者中,TRUS发现异常病灶区患者103例,病理确诊前列腺癌48例占46.6%。TRUS未发现异常病灶区患者36例,病理确诊前列腺癌7例占19.4%。结论经直肠超声择点穿刺前列腺活检可提高前列腺癌的阳性率,减少穿刺针数,缩短穿刺时间,减轻患者穿刺痛苦和并发症的发生,有较好的临床实用价值。

Evaluation of selected prostate puncture biopsy by transrectal ultrasound

Objective To discuss clinical application of selected prostate puncture biopsy by transrectal ultrasound（TRUS）.Methods 139 patients suspected to have prostate cancer were examined by transrectal ultrasound.The abnormal focal areas were located as puncture point and the number of punctures was confirmed too.Afterwards all patients underwent selected prostate puncture biopsy by transrectal ultrasound.Results Among the 139 patients,103 were identified as TRUS（＋）,and 48 cases were pathologically proved as cancer（46.6%）.In 36 TRUS（-） cases,7 were pathologically proved cancer（19.4%）.Conclusion The selected prostate puncture biopsy by transrectal ultrasound could improve the detection rate of prostate cancer,reduce the puncture frequency and surgical duration,and have less pain and fewer complications.It has good clinical practical value.
