文拉法辛缓释剂与帕罗西汀治疗难治性抑郁症对照研究

目的探讨文拉法辛缓释剂与帕罗西汀治疗难治性抑郁症的临床疗效和安全性。方法将符合入组标准的80例患者随机分为文拉法辛组和帕罗西汀组各40例。于治疗前和治疗后第1、4、8周末采用汉密尔顿抑郁量表﹙HAMD﹚17项评定疾病严重程度和疗效,采用不良反应量表﹙TESS﹚评定药物不良反应。结果两组治疗后汉密尔顿抑郁量表评分均较治疗前有显著下降(P0.01),且随着治疗时间的延续均呈持续性下降;文拉法辛组治疗第一周末较帕罗西汀组下降更显著(P0.01),其他时点评分两组均无显著性差异(P0.05)。两组不良反应均较轻微,多发生在治疗初期,并随着治疗的延续逐渐减轻或缓解。结论文拉法辛缓释剂与帕罗西汀治疗难治性抑郁症效果均较显著,安全性高,依从性好;但文拉法辛缓释剂起效更快,值得临床推广应用。

Venlafaxine Rretarder and Paroxitine for Refractory Depression: A Controlled Study

Objective To explore the clinical efficacy and safety of venlafaxine retarder and paroxitine in the treatment of refractory depression. Methods We divided 80 cases of refractory depression randomly into venlafaxine group and paroxetine group for 8 week double-blind treatment. At 1, 4 and 8 weeks before and after treatment, we evaluated the efficacy by using Hamilton Depression Scale （HAMD） and the adverse effects by using treatment Emergent Symptom Scale （TESS）. Results After treatment, the HAMD scores of both groups were lowered more significantly compared with pre-therapy （P〈0.01）,with a persistent decline as the treatment time lasted. The decline in the venlafaxine group was more significant than that in the paroxetine group at the end of the first week （P〈0.01）,with no significant differences in the other time points （P〉0.05）. Adverse reactions of both groups were mild,mostly happened in the initial stage of treatment and lessened or relieved gradually with treatment lasting. Conclusion Both venlafaxine retarder and paroxetine have a notable effect, with higher safety and better compliance in the treatment of refractory depression, but the former acts more rapidly, worthy of being popularized.
