胎盘生长因子和脂联素联合检测在AMI中的临床意义

摘    要:目的:探讨急性心肌梗死(AMI)患者胎盘生长因子(placental growth factor,PLGF)和脂联素(adiponectin,APN)含量的变化及联合检测的临床意义。方法:采用酶联免疫法测定52例AMI患者和70例健康对照组血清PLGF和血浆APN的水平,采用全自动生化分析仪检测患者血清肌红蛋白(Mb)和肌钙蛋白T(cTnT)。结果:AMI患者在<6h、12h、24h采血,PLGF水平在6h内升高不明显,与对照组相比差异无统计学意义(P>0.05);12h开始增高,与对照组相比差异有统计学意义(P<0.05);24h明显升高,与6h内及对照组相比差异均有统计学意义(P<0.01);APN水平在6h内即有明显降低,与对照组相比差异有统计学意义(P<0.05);24h显著降低,与6h内及对照组相比差异均有统计学意义(P<0.01);经直线相关分析:PLGF水平与Mb、APN无相关性(P>0.05),与cTnT呈正相关(r=0.67,P<0.05);APN与Mb呈负相关(P<0.05)。结论:PLGF及APN分别是AMI发生的独立危险因素,早期联合检测PLGF和APN有利于对AMI早期心肌坏死程度的评估及危险终末事件发生率的预测。

Abstract:Objective:To explore the variation of serum placental growth factor(PLGF) and plasma adiponeetin (APN) levels and the clinical significance of their combined detection in patients with acute myocardial infarction. Methods. The level of serum PLGF and plasma APN were assayed by enzyme-linked immunosorbent assay in 52 AMI cases and 70 healthy cases, while serum myoglobin and troponin T were detected by auto biochemical analyzer. Results: As compared with control group, there was no significant increase of serum PLGF levels within 6 h in AMI group (P〉0.05) ,PLGF began to increase at 12 h (P〈 0.05), and there was significant increase at 24 h in AMI group as comparing with the level at 〈6h and the level in control group(P〈0.01). The level of APN was significantly decreased at 6 h and was obvious lower in the control group (P〈0.05) ,at 24 h, the decrease in AMI group was significant lower that of the 6 h and that of the control group(P〈0.01). Linear correlation analysis showed that the level of the PLGF had no correlation with Mh and APN(P〉0.05)but had a positive correlation with cTnT(r=0.67,P〈0. 05). APN had a negative correlation with Mb(r= -0. 73, P〈0.05). Conclusion:PLGF and APN were independent risk factor for AMI. Early combined monitoring of the PLGF and APN levels can help in evaluating the extent of myocardial necrosis and the prediction of dangerous terminal case occurrence.
