乳腺癌保乳术后淋巴结转移的高危因素分析

目的:评价乳腺癌保乳术后发生淋巴结转移的高危因素。方法:研究178例乳腺癌患者的临床及病理资料,将其中发生淋巴结转移的15例与无淋巴结转移病例进行临床与病理对比分析,寻找相关危险因素。结果:转移组与无转移组相比,患者年龄较轻,肿瘤直径较大,E-cadherin表达较低,HER-2表达较高。而病理类型、组织学分级、VEGF-C、NM23、MMP-2、Ki-67等方面差异无统计学意义。结论:年龄≤37岁、肿瘤最大径>3.0cm、E-cadherin表达低、HER-2阳性是保乳术后乳腺癌淋巴结转移高危因素。因此,对于此类患者应该加强定期复查,警惕乳腺癌转移。

Analysis of Relative Risk Factors of Lymphatic Metastasis after Conservative Operation in Breast Cancer Women

Objective：To explore the relative risk factors of lymphatic metastasis after breast cancer conservative operation. Methods： There were 178 cases of breast cancer conservative operation from January 2000 to December 2005, and 15 cases occurred lymphatic metastasis among it. The clinical and pathological analysis was performed in these cases to find the relative risk factors. Results. Between non-metastasis group and metastasis group, there were statistical by significant difference in the patient＇s age, tumor size, low expression of E-cadherin and high expression of HER-2, but there were no statistically significant difference in pathological type, histological grade, expression of VEGF-C, NM23, MMP-2, Ki-67. Conclusion. For the breast cancer conservative operation, the various risk factors should be assessed, which would cause lymphatic metastasis individually and synthetically, and for such patients should strengthen regular review.
