骨源性碱性磷酸酶与佝偻病早期诊断及治疗关系

目的研究骨源性碱性磷酸酶在维生素D缺乏性佝偻病的早期诊断及治疗中的应用价值。方法对门诊6个月～3岁简易诊断为佝偻病的患儿采静脉血，分别检查血清钙、磷、碱性磷酸酶（AKP）及骨源性碱性磷酸酶（NEAP）。分析上述检验值在佝偻病诊断中灵敏度，应用SPSS10．0统计软件对AKP和NBAP进行相关性分析，NBAP≥200U／L者给予胆维丁乳治疗并观察治疗效果。结果AKP和NBAP具有相关性，但在诊断佝偻病时NBAP的灵敏度更高，对NBAP≥200U／L者给予胆维丁乳治疗1～3次后，患儿多汗、夜惊症状在给予治疗后2周消失，3个月后骨性变化有好转，上述生化指标均恢复正常。结论NBAP在小儿佝偻病诊断中灵敏度高，特异性强，是佝偻病早期诊断的可靠指标，并可较好的指导佝偻病治疗。

Relation between Bone AKP and Early Diagnosis of Rachitis

Objective To discuss the application value of bone AKP in early diagnosis and treatment of vitamine D insufficiency rachifis, Methods The serum calcium, phostate, AKP and NBAP were detected in 6 monts to 3 years old children rachitis patient. The sensitivity of data above was analyzed and SPSS10.0 statistics was taken to compare AKP and NBAP relativity. The vitamin D3 was given for those NBAP more than 200 u/L. Results There was relationship between AKP and NBAP with high sensitivity of NBAP. The patient whose NBAP more than 200 u/L was taken vitamin D3 for 2 weeks, their symptoms disappeared and bone changed after 3 months and biochemical data recovered. Conclusion NBAP might have high sensitivity in early diagnosis of rachitis with high specificity. It might be reliable data to diagnose and treat this disease.
