化脓性关节炎后遗髋关节畸形的全髋关节置换

目的探讨化脓性关节炎后遗髋关节畸形的全髋关节置换术疗效。方法对2001年4月~2005年2月,将8例接受单侧全髋关节置换术的化脓性关节炎后遗髋关节畸形的患者作为研究对象。患者手术时年龄30~54岁,平均38·4岁。术前短缩2·5~5cm。术中髋臼假体置入真臼中,以重建髋关节旋转中心。结果术后双下肢等长(双下肢相差在1cm内)7例,1例短缩1·5cm。Harris评分由术前平均54分增加到87分。无感染,未见假体松动及植骨块吸收、塌陷。结论化脓性髋关节炎后遗髋关节畸形,全髋关节置换术是一有效的治疗方法。

Total Hip Arthroplasty for the Abnormal Hip after Suppurative Arthritis

Objective To investigate the effect of the total hip arthroplasty for the treatment of the abnormal hip after suppurative arthritis.Methods Eight patients underwent the unilateral primary total hip arthroplasty for the abnormal hip after the suppurative arthritis from April 2001 to February 2005.The ages of the patients at the surgery ranged from 30 to 54 years old (mean 38.4 years old).The preoperative limb-length discrepancy ranged from 2.5 to 5 cm.During the surgery, the acetabular cup was implanted into the true acetabulum to reconstruct the hip center of rotation. Results After surgery,7 patients had equal limb length with the discrepancy less than 1cm and one patient had the limb-length discrepancy of 1.5 cm.According to the Harris hip score, the average hip score increased from 54 preoperatively to 87 at the final review.The hip joint infection, hip prostheses loosening,autografts absorption and collapse occurred in no patients.Conclusion The total hip arthroplasty is a effective method for the treatment of the abnormal hip after the suppurative arthritis.
