支气管内膜结核纤维支气管镜下的形态表现

目的研究纤维支气管镜下支气管内膜结核病人（以下简称纤支镜）下形态改变。方法分析临床确诊的支气管内膜结核患者35例，其中女性20例、男性15例，对其纤支镜下形态改变进行总结、归纳。结果对临床上确诊的支气管内膜结核病人纤支镜下形态学改变进行分析，归纳为以下7种类型的表现：粟粒型5例、干酪样坏死型7例、充血水肿型6例、疤痕狭窄型4例、肿瘤型3例、溃疡型3例和非特异性支气管炎型7例。结论纤支镜下支气管内膜结核7种类型的表现反映了该病自然病程中不同发展阶段的形态学改变，纤支镜检查对支气管内膜结核具有较大诊断价值，临床上怀疑支气管内膜结核时应及时行纤支镜检查。

The morphologic change of encobronchial tuberculosis under fiberoptic bronchoscope

Objective To analyze the morphologic change of endobronchial tuberculosis (EBTB) lesion under bronchoscopy. Methods The bronchoscopic manifestations of EBTB were evaluated in 35 patients. The manifestations under bronchoscope were classified into seven subtypes. Results According to bronchoscopic features, 35 cases of EBTB were classified into seven subtypes: 5 of granular, 7 of active caseating, 6 of edematous-hyperemic, 4 of fibrostenotic, 3 of tumorous, 3 of ulcerative, and 7 of nonspecific bronchitic. Conclusion We suggest that the different subtypes of EBTB may reflect morphologic changes of different periods in the natural course of EBTB lesions. Fiberoptic bronchoscopy plays an important role in the diagnosis for the patients who are suspected to have tuberculosis.
