Symptom clusters in 1631 patients with lung cancer undergoing chemotherapy

Objective To investigate the incidence of symptom clusters and influencing factors in patients with lung cancer undergoing chemotherapy.Methods A total of 1631 patients with lung cancer undergoing chemotherapy were selected at a specialized first-class cancer hospital from January 2017 to July 2017.They were investigated using the Chinese version of M.D.Anderson Symptom Inventory(MDASI).Then they were followed-up using telephone and clinical symptoms were recorded and analyzed.Results The top three common symptoms were fatigue(57.4%),poor appetite(45.1%)and nausea(42.9%).The severity scores of fatigue,poor appetite,nausea,sleep disturbance,dry mouth and vomiting symptoms P75 all reached 5.The KMO test value was equal to 0.861 and P value for Bartlett's test of sphericity was 0.00.The exploratory factor analysis was used to extract the symptom clusters for the factors whose characteristic value was greater than 1.0,and factors whose factor score coefficient was less than or equal to 0.4 were removed.Finally,a total of 3 symptom clusters were extracted,namely the gastrointestinal symptoms clusters,the mental and psychological symptoms clusters,and the disease behavioral symptoms clusters.The accumulative variance contribution rate was 53.9%.Spearman correlation analysis was performed for the 3 symptom clusters and the number of symptoms occurrence,overall score of symptom experience and overall score of symptom interference,respectively.The r value of>0.5 was up to 0.952,and P value was<0.01 in each corre-lation analysis suggesting a positive correlation.Conclusion Nurses should pay more attention to symptom clusters using comprehensive interventions to decrease the degree of symptom distress and maximize the effect of interventions.
