洛铂腔内注射治疗恶性胸腔积液的临床观察

目的观察洛铂胸腔灌注与顺铂胸腔灌注治疗恶性胸腔积液的疗效及安全性。方法5年内治疗恶性胸腔积液146例,其中洛铂组（A组）75例,顺铂组（B组）71例,用粗孔中心静脉导管行胸腔留置,至少行腔内给药二次以上。A组给洛铂40mg/m2＋50ml氯化钠注射液胸腔灌注,B组给顺铂40mg/m2＋50ml氯化钠注射液胸腔灌注。结果A组总有效率明显高于B组,A组有效率为64.00%,而B组有效率仅为49.30%,差异有统计学意义（P〈0.05）。在毒副反应上,胃肠道反应方面,A组明显低于B组,差异有统计学意义（P〈0.05）。结论洛铂可做为胸腔灌注药物之一,且疗效及毒副作用方面,均明显优于传统药物顺铂。

Clinical observation on the efficacy of Ioplatinum injection in treatment of patients with malignant hydrothorax

Objective To observe the efficacy and safety of loplatinum intrapleural infusion combined with cisplatin in- trapleura/perfusion in the treatment of malignant pleura/ effusion. Methods The 146 malignant pleural effusion patients treated for five years were divided into loplatinum group （ group A） consisted of 76 cases and the cisplatin group （ group B） com- prised of 70 cases. The patients in group A and B were treated with loplatinum at dose of 40mg/m2＋50ml and cisplatin at dose of 40mg/m2＋50ml through coarse pore central venous catheter indwelling pleura/cavity at least two times. Results The total effective rate of group A was 64.00%, significantly higher than that of group B（ 49.30%）,showing significant difference （ P 〈 0.05）.The toxicity and gastrointestinal reactions in group A were lighter than that in group B（ P 〈0.05）. Conclusion Lobaplat- in is superior to cisplatin in efficacy and adverse reactions.
