术后椎间盘炎的诊治体会

目的总结7例腰椎间盘突出症患者术后继发椎间盘炎的临床特点，探讨其早期诊断和治疗方法．方法我院1990年6月至2007年8月手术治疗腰椎间盘突出症患者术后继发椎间盘炎7例，其中男2例，女5例，年龄35～63岁，平均42岁．L4．，5例，ks，2例．行椎间盘摘除术后第3～11天又出现腰痛，平均6d．7例均做血沉，采用卧床、腰部制动，抗生素和非甾体抗炎药等治疗．结果本组先期6例治疗后症状均消失无复发，腰椎活动度良好，均恢复正常生活和工作．近期1例术后效果优，第5天腰部出现轻微疼痛，加重并寒战高热，最终因感染中毒性休克死亡．结论腰椎间盘突出症术后继发椎间盘炎经保守治疗症状进行性加重者，宜早期积极手术干预．

The Experience in Diagnosis and Treatment of Surgical Discitis

Objective To summarize the clinical characteristics of the secondary surgiacl discitis in 7cases of lubar intervertebral disc protrusion, and discuss the diagnosis and treatment in early stage. Method 7cases of secondary surgiacl discitis after the operation of lubar intervertebral disc protrusion were collected from June, 1990 to August, 2008, include 2 male cases, 5 female case, and the age ranged from 35 - 63, 42 years in average. There were 5 cases in L4,5, 2 in L5S1. Lumbago apppeared on 3rd to 1 lth day after discectomy, 6 day in average. Results The symptom disappeared after integratedtreatment in 6cases, no symptom recurrence was found and the range-of-motion of lumbar was good. The surgical effect in 1 case was good, and minor pain appeared in 5th day, then aggravated and combined with CFNS and hyperpyrexia. At last, died in toxic shock. Gonclusion The patients with progressing symptoms of secondary surgiacl discitis after the operation of lubar intervertebral disc protrusion should recieve surgical operation positively in early stage.
