塞来昔布联合督脉灸、揿针治疗活动期强直性脊柱炎25例临床观察

目的:观察塞来昔布联合督脉灸、揿针治疗活动期强直性脊柱炎患者的临床疗效。方法:将50例强直性脊柱炎患者随机分为治疗组和对照组,每组25例。对照组给予塞来昔布口服联合督脉灸(每次30 min,每日1次)治疗;治疗组在对照组治疗基础上联合揿针(每次6 h,每日1次)治疗。2组均以2周为1个疗程。观察2组临床疗效,以及治疗前后红细胞沉降率(ESR)、C反应蛋白(CRP)、疼痛视觉模拟评分法(VAS)评分、Bath强直性脊柱炎功能指数(BASFI)、Bath强直性脊柱炎活动指数(BASDAI)和Bath强直性脊柱炎测量指数(BASMI)。结果:治疗组临床缓解0例,显效17例,有效6例,无效2例,总有效率为92.00%;对照组临床缓解0例,显效6例,有效10例,无效9例,总有效率为64.00%。2组比较,差异有统计学意义(P<0.05)。治疗后,2组ESR、CRP、VAS评分、BASFI、BASDAI、BASMI较治疗前均有改善(P<0.05),且治疗组改善程度明显优于对照组(P<0.05)。结论:塞来昔布联合督脉灸、揿针治疗能明显改善活动期强直性脊柱炎患者的脊柱关节功能,有效减轻关节疼痛,提高脊柱关节活动能力。

Clinical Observation on 25 Cases of Active Ankylosing Spondylitis Treated by Celecoxib Combined with Du Meridian Moxibustion and Pressing Needle

Objective:To observe the clinical efficacy of celecoxib combined with Du Meridian moxibustion and pressing needle in the treatment of active ankylosing spondylitis.Methods:Fifty patients with ankylosing spondylitis were randomly divided into a treatment group and a control group,25 cases in each group.The control group was treated with celecoxib orally combined with Du Meridian moxibustion(once a day,30 minutes for each time),while the treatment group was treated with pressing needle based on the treatment for the control group(once a day,6 hours for each time).Both groups were treated for 2 weeks as a course of treatment.The clinical efficacy,erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),visual analogue scale(VAS),Bath Ankylosing Spondylitis Functional Index(BASFI),Bath Ankylosing Spondylitis Disease Activity Index(BASDAI)and Bath Ankylosing Spondylitis Measurement Index(BASMI)before and after treatment were observed.Results:In the treatment group,no case had clinical remission,17 cases were markedly effective,6 cases were effective and 2 cases were ineffective,and the total effective rate was 92.00%;while in the control group,no case had clinical remission,6 cases were markedly effective,10 cases were effective,9 cases were ineffective,and the total effective rate was 64.00%.The difference between the two groups was statistically significant(P<0.05).After treatment,ESR,CRP,VAS,BASFI,BASDAI and BASMI of the two groups were improved compared with those before treatment(P<0.05),and the improvement degree of the treatment group was significantly better than that of the control group(P<0.05).Conclusion:Celecoxib combined with Du Meridian moxibustion and pressing needle can significantly improve the spinal joint function of patients with active ankylosing spondylitis,effectively reduce joint pain and improve the spinal joint activity.
