超声、CT检查中膝关节骨关节炎影像学表现及其诊断价值对比

目的对比超声、CT检查中膝关节骨关节炎影像学表现及其诊断价值。方法选取我院2017年1月至2018年12月收治的膝关节骨关节炎患者46例,收集46例患者的临床资料、影像学资料,所有患者均进行超声、CT检查,并对其结果进行诊断正确率对比。结果CT扫描对膝关节骨关节炎患者的关节软骨变薄、滑膜改变、半月板改变、软组织改变、关节腔改变、关节积液的诊断符合率分别为82.60%(19/23)、91.30%(21/23)、60.80%(14/23)、73.91%(17/23)、65.21%(15/23)、78.26%(18/23),超声的扫描膝关节骨关节炎患者的关节软骨变薄、滑膜改变、半月板改变、软组织改变、关节腔改变、关节积液的诊断符合率分别为65.21%(15/23)、69.56%(16/23)、34.78%(8/23)、56.52%(13/23)、43.47%(10/23)、60.86%(14/23),CT扫描的诊断正确率明显高于超声扫描组,差异无统计学意义(P>0.05),CT扫描对膝关节骨关节炎患者的关节软骨回声诊断符合率为78.26%(18/23),超声的扫描诊断符合率为47.82%(11/23),CT扫描的诊断正确率显著高于超声扫描组(P<0.05)。结论超声与CT对膝关节骨性关节炎患者的诊断均是有效的,但CT扫描对患者更有利于分析疾病的病变部位暴露,对临床治疗方案的制定更具有指导意义。

Comparison on Imaging Features and Diagnostic Value of Knee Osteoarthritis in Ultrasound and CT Examination

Objective To compare the imaging features and diagnostic value of knee osteoarthritis in ultrasound and CT exa mination.Methods 46 patients with knee osteoa rthritis who were admitted to our hospital from January 2017 to December 2018 were selected.The clinical data and imaging data of 46 patients were collected.All patients underwent ultrasound and CT examinations.And the correct rate of diagnosis was compa red.Results The diagnostic accu racy of CT scan on thinning of a rticular ca rtilage,synovial changes,meniscus changes,soft tissue changes,joint cavity changes,and joint effusion in patients with knee osteoarthritis was 82.60%(19/23),91.30%(21/23),60.80%(14/23),73.91%(17/23),65.21%(15/23),78.26%(18/23),and diagnostic accuracy of ultrasound was 65.21%(15/23),69.56%(16/23),34.78%(8/23),56.52%(13/23),43.47%(10/23),and 60.86%(14/23).The diagnostic accu racy of the CT scan was significantly higher than that of the ultrasound,and the difference was not statistically significant(P>0.05).The diagnostic accu ra cy of CT scan on the echo of a rticula r ca rtilage in patients with knee osteoarthritis was 78.26%(18/23),and the diagnostic accuracy of ultrasound was 47.82%(11/23).The diagnostic accuracy of CT scan was significantly higher than that of ultrasound(P<0.05).Conclusion Ultrasound and CT a re effective in diagnosing knee osteoa rthritis patients,but CT scan is more more beneficial to analyze the location of the lesions of the disease in patients,and has more guiding significance for the formulation of clinical treatment plans.
